Move Your Fulcrum: NPSGs

From the Centers for Disease Control......

The factors necessary for change include:

  1. dissatisfaction with the current situation,
  2. perception of alternatives, and
  3. recognition, both at the individual and institutional level, of the ability and potential to change.

Although the latter implies education and motivation, the former two necessitate a system change.  Among the reported reasons for poor adherence with recommendations, certain ones are clearly associated with the institution or system:

  1. lack of institutional priority for the goal,
  2. administrative sanctions, and
  3. a safety climate.

Although all of these reasons would require a system change in the majority of institutions, the third requires management commitment, visible safety programs, an acceptable level of work stress, a tolerant and supportive attitude toward reported problems, and belief in the efficacy of preventive strategies.

Most importantly, an improvement in practices requires

  1. questioning basic beliefs,
  2. continuous assessment of the group (or individual) stage of behavioral change,
  3. intervention(s) with an appropriate process of change, and
  4. supporting individual and group creativity.

Because of the complexity of the process of change, single interventions often fail. Thus, a multimodal, multidisciplinary strategy is likely necessary.    -Centers for Disease Control – Guidelines for Hand Hygiene in Health-care Settings 2002

 

Regarding any change initiatives around NPSGs.....

Compliance targets are set but we all know that we should offer the best care because it is the right thing to do. Somehow, we lose sight of that and we take shortcuts that may impact the health of our patients and the bottom line of our organization.

Our Fulcrum Program for NPSGs goes beyond teaching your staff what is the right or wrong behavior. We help the staff member get his or her head into the game. Each team member will be a leader in this effort. Practicing new behaviors necessitates a change in you. For many of us, change is an abstract notion. Where do we begin? How uncomfortable will it be? How much effort does it take? What do I want to change? What if no one else around me changes? Is it worth the effort?

It is worth the effort if you are not getting the outcomes you really want.

In the Discover My Base phase you will examine what you are bringing to your work life and your personal life right now? You will look at some of your barriers to learning.

In Act on Values you will add who you are. Where do your values originate? How do they show up everyday of your life? Are stress factors associated with your choice to walk your talk or to be walked upon, in fact, to sell out?

In Become Authentic - Act on Choices  you will recognize the moment you choose to comply or not, or a moment you choose to talk to a colleague about complying or not.  You can then act on the choices.

And in Master Personal and Team Accountability you will play an important role in supporting a culture in which it is safe to do the right thing for the patient, the team, and the organization.
